Care Management Support Coordinator II (Must reside in San Antonio, TX)

As a Care Management Support Coordinator II , you will support administrative care management activities, including member outreach, inbound calls, and service scheduling. You'll be the go-to contact for members, providers, and staff, resolving issues while ensuring documentation adheres to regulatory standards.
Key Responsibilities:
Member Support: Connect members to essential health plans and community resources to improve their care experience.
Front-line Support: Act as the first point of contact for member and provider inquiries, explaining care plan procedures and protocols.
Administrative Support: Assist with member onboarding, send welcome letters, educational materials, and other necessary correspondence.
Documentation: Maintain non-clinical member records, ensuring compliance with state and regulatory requirements.
Social Determinants of Health (SDOH): Use local knowledge to make referrals addressing SDOH needs and enhance member outcomes.
Additional Duties: Provide exceptional customer service while adhering to organizational policies and standards.
Job Requirements:
Location: Must reside in the San Antonio, TX, area.
Training: 3-4 weeks of in-person training, followed by hybrid work (may need to return to San Antonio office if needed).
Experience:
High call volume experience - Preferred.
Previous appointment scheduling experience - Preferred
Strong customer service background - Required
Skills: Basic computer skills and typing proficiency - Required
Education: High school diploma - Required
Job Details:
Schedule: Full-time, Monday to Friday, 8:00 AM - 5:00 PM
Compensation: $18.50/hour, plus comprehensive benefits
Benefits: Health, dental, vision, 401k program
Employment Type: Temp-to-Hire Opportunity
This is a hybrid position , with 3-4 weeks of in-person training followed by remote work, though you may be required to return to the San Antonio office based on business needs.
