Position: Home Health Care Scheduler Location: Holland, Michigan About Us At Continuum Home Health Care, we're dedicated to providing exceptional in-home care. The Role This is NOT your typical medical office scheduling position! Home health care scheduling involves managing a constantly evolving puzzle where:

• Caregivers call off with little notice
• Client needs change unexpectedly
• Emergencies require immediate rescheduling
• You must ensure no client goes without essential care

You'll need to think on your feet, quickly identify solutions, and coordinate replacement caregivers—often within minutes—to maintain uninterrupted care for vulnerable clients who depend on our services. Requirements (Non-Negotiable)

• Prior home health care scheduling experience is a plus
• Experience with home health care software is a great advantage
• Knowledge of AxisCare is a significant advantage
• Exceptional problem-solving under pressure
• Ability to manage multiple urgent situations simultaneously

Schedule

• Monday-Friday, 8:00 AM - 4:30 PM
• On-call rotation: One weeknight per week and one weekend per month (answering calls and making schedule changes)

Compensation

• Starting at $19/hour
• Potential for increases based on experience
