Nurse Practitioner – Territory-Based (California)

Full-Time | Monday–Friday | No Weekends or Holidays

Location: Morgan Hill/South San Jose, California 
Applicants must reside in Northern California, hold an active California Nurse Practitioner license, and be willing to travel regularly within assigned territories.

About the Role: Where Productivity Meets Purpose

As a Territory-Based Nurse Practitioner at PrimoMD, you’ll be out in the field, visiting multiple facilities within your assigned region to deliver assessments to seniors, often among the most underserved patients in our healthcare system.

This is not a low-volume role. You’ll work full, structured days seeing multiple patients, each of whom relies on your clinical insight, empathy, and expertise.

You will:

• Travel between assigned facilities within your territory.
• Work Monday through Friday during regular business hours. 
• No weekends- no holidays.
• Partner with a multidisciplinary team to create and implement effective care plans.
• Document care accurately and efficiently in a secure system.

What to Expect:

• In-person patient visits with seniors managing chronic conditions.
• Full 8-hour workdays, Monday through Friday.
• A fast-paced, structured, and patient-centered care environment.
• Mileage reimbursement for travel within your territory.

Who We’re Looking For:

• Active California Nurse Practitioner license (Required).
• Board-certified in Family Medicine, Geriatrics, or a related specialty.
• Minimum 2 years of clinical experience (senior care preferred).
• Organized, self-motivated, and disciplined.
• Able to manage a high-volume patient load with consistency and compassion.
• Comfortable driving between a few facilities each day.
• A clear, empathetic communicator with strong clinical judgment.
