Job Description:

Phoenix Home Care & Hospice is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide compassionate, skilled 1:1 care in the homes of our clients in Hannibal, MO . We offer full-time and part-time positions with day and evening shifts available, providing you the flexibility to work around your personal needs while delivering high-quality care.

Job Responsibilities:

• Follow the individualized plan of care developed with the physician
• Monitor, document, and report the patient’s condition using electronic charting
• Educate and provide support to the client and their family
• Attend team meetings and training sessions as required

Qualifications:

• Active State of Missouri or Compact Nursing License (LPN or RN)
• Excellent communication and critical thinking skills
• Compassionate, professional, and positive attitude
• Ability to work independently and manage time effectively
